How-to guide
How to make a talent show registration form
What a talent show registration form should ask, and how to get one online in minutes with parent consent, a photo and a registration number for every entry.
A talent show registration form needs to collect enough to choose the right contestants and reach them later — without being so long that people give up on their phones.
With MJX Events Talent Hunt you do not build the form: every talent hunt gets a ready one on its own page, with the right questions and consent built in.
What the form asks
Who they are
Name, age, city and a mobile number for WhatsApp.
Their talent
From the talents you ticked — singing, dance, art and more.
A photo
So you can put a face to every entry when you shortlist.
Consent
The contestant terms, and a parent or guardian’s consent for anyone under 18.
How it works
- 1
Create your talent hunt on MJX Events (free).
- 2
In Settings, tick the talents you want on the form and set the last date to register.
- 3
Check the details contestants see: your organisation’s name, the city for disputes and your contact email.
- 4
Open registrations. Your form is live on its own link, with a QR code for posters.
- 5
Share the link. Every entry arrives in one list with a registration number, and you can download them for Excel.
Questions and answers
- What should a talent show registration form include?
- Name, age, city, a mobile number, the talent, a photo, and consent — including a parent or guardian’s consent for anyone under 18.
- Can I use a general online form instead?
- You can, but you then have to shortlist, message, judge and add up marks by hand. MJX Events Talent Hunt does those steps on the same page as the form.
- Do contestants get a registration number?
- Yes. Every entry gets one, and it is in the WhatsApp message when they are shortlisted.
